1. Visa application form, duly signed & dated with attached passport-size photo.
2. Passport, showing travel history including previous Schengen Visa
3. Passport bio-page Copy with signature.
4. Travel Insurance.
5. Round-trip airline ticket reservation
6. Hotel reservations
7. Marriage contract certified by the Philippine Statistics Authority (PSA) ( IF married)
8. Certificate of employment.
9. Approved Leave form.
10. 6 months Bank statement
11. 6 months Credit card statement (Additional supporting document)
12. Proof of social security contributions.
13. Copy of Real Estate property (title).
14. Copy of Car Certificate of Registration.
